Faithbridge intends to continually update our website. We would like to use photos of volunteers on that website. If you would like to give us permission to use your child's picture or any picture with them in it, please read and complete the form below. If you do NOT want to give permission to use any picture of your child, please let a Kid Life staff member know before they begin serving.

I grant permission to Faithbridge of Racine, Wisconsin to use photographs of me or my likeness, for use on their website or other church publications without notifying me. I hereby waive any right to inspect or approve the photographs, publications, or electronic matter that my may be used in conjunction with them now or in the future, whether that use is known to me or unknown. I hereby agree to release and hold Faithbridge from and against any claims, damages or liability arising from or related to the use of the photographs of me or my likeness. I have read this release before signing below, and I fully understand the contents, meaning and impact of this release. I understand that signing this release is entirely an act of my own free will.
