Student Conduct Agreement
We expect each student to conform to these rules of conduct.
No possession or use of alcohol, drugs, tobacco or vapes.
No fighting, weapons, fireworks, lighters, or explosives
Participation with the group is expected.
Respect property
Respect one another, staff, and adult leaders
Respect and comply with event schedules
Students who fail to comply with these expectations may be sent home at their parents’ expenses.
I, the student, have read the rules of conduct and agree to abide by the stated personal limitations and code of conduct.
Medical and Transportation Waiver
By signing below, the parent or guardian agrees to the following:
1. I will not hold Faithbridge Church, its employees, volunteers or anyone acting on its behalf, responsible or liable for injury occurring to the named child(ren) in the course of travel.
2. I authorize Faithbridge Church to transport and to obtain, through a physician of its own choice, any emergency medical care that may become reasonably necessary for the person in the course of travel, and agree to accept the cost of the transportation and/or treatment by medical personnel or facility.
3. I accept full responsibility and hereby grant permission for my minor child(ren) to travel with Faithbridge Church.
Payment Options:
1) Pay now with a charge card.
2) Pay with cash or check at the Church Office. Check's payable to Faithbridge Church.
3) Paying for multiple students.**
Student's spot is reserved when payment is received.
** (If you would like to pay for mutliple students, select Option 3 above for each student and submit the forms. To pay by charge, go to our church website, the "Give" page faithbridgewi.com/give and select "Give to Student Life: Winter Retreat", and pay the full amount. If paying by cash or check, deliver the full amount to the office. Contact Sherri in the office if you have more questions.)
The full amount for the weekend is $170 per camper and includes 2 nights and 5 meals.
For our Student Life students:
Our Dessert Derby raises money to help offset this cost. Therefore, the fee per student is $50. If any student needs payment assistance, please contact Rachel, Sherri, Annelise or Logan directly. We want our students that are interested in growing deeper in their faith with their peers to have this experience.
